Could you please confirm if either I am looking the wrong place or the answer is incorrect.
For Question 2:
The minimum radion navigation aid requirement for aircraft with a MTOW bwlow 4600 kg for IFR operations outside controlled airspace in the charter category is
and the answer on page 2.35 is given as one VOR or one ADF. My AIP seems to suggest that a C145a or C146a GNSS is also a possibility.

It seems you may have an older version of the IREX book. It is true that a C145 or C146 GNSS would do the trick. In any case, if a question only gave you one VOR or one ADF as one of the choices and did not mention anything about RNAV, that choice would still be the answer to that question. That is, out of the choices given, one VOR or one ADF would be the answer to select.
