i just converted my lic to CPL Australian. first you need to call CASA fill form no. 61-4A than wait for assessment. following documents need to be sent;

1. License copy attested
2. last five pages of log book attested
3. medical first class
4. same time apply for ASIC
5. get your EPC (English Proficiency Check) original need to be with form 61-4a
6. start studying BOB Tait air law and Human Factors you have to give these two theory exams
7. air law 80% passing (thanks to BOB tait I got 98%)
8. Human Factor 70% passing ( again bob tait i got 100%)
9. and you need to fill your identity form with last six months current photo (again photo attested)
10. let your other license provider know that an inspection going to come for cross check
11. and make payment online for assessment and ASIC provide copy

if your are registered on self service at casa website you can download these from there or simply you can download from casa website ;

Form 61-4A - CASA Flight Crew Licence Application on basis of Overseas Civil Qualifications (Part A)
and all documents need to be sent in hard copy.
checklist is on front page of this form.
please do call them check with them for any other requirement
casa is going through quite a huge change in licensing so be sure before you dispatch any documents
almost waiting period as per oct.2015 two to three months for assessment. (my rough estimate but it may change according to work load)
